Fee Schedule

Our fees are set using the Optum National Fee Analyzer. Medicaid and Medicare fees are set by the respective agencies. Self-pay patients are required to provide financial information if they would like to be considered for the sliding fee scale which could reduce the amount that they have to pay for services.
